Alberta bans puberty blockers and cross-sex hormones for gender reassignment under 16. At 16 and 17, a child may still start them if a parent or guardian, a physician, and a psychologist all approve. A minor who had already started, and a minor with a disorder of sex development, are excepted. Sex-reassignment surgery has been banned for anyone under 18 since December 2024.

The limit is on prescribers. They may not prescribe puberty blockers or cross-sex hormones for gender reassignment to a child under 16. At 16 and 17, a parent or guardian, a physician, and a psychologist must all approve. It is not a separate public drug-plan list.

From 1 September 2025, a girls' or women's division for athletes 12 and older is limited to athletes whose sex at birth is female. Sex at birth means the sex on the birth registration.

If a student asks staff to use a new name or pronouns tied to gender identity, the school must tell the parent. For a student under 16, staff may not use that name or those pronouns unless the parent agrees.

For girls' and women's sport, sex at birth means the sex written on the birth registration. Alberta has no general law that defines male and female for every other purpose.

A male who identifies as a woman can be housed in a women's federal prison, even if his body is male and his identification still says male, unless a health or safety problem cannot be resolved. A federal count of 99 gender-diverse prisoners, from December 2017 to March 2020, found that 62 percent were males who identify as women. At the end of that count, 67 percent of that group were still in men's prisons. The federal women's prison in Alberta is Edmonton Institution for Women.

Bill 9, which received royal assent on 11 December 2025, says Alberta's medical, school, and sport rules for children apply even where the Charter of Rights would otherwise limit them.

Sex-reassignment surgery on minors was prohibited in December 2024. The sport law took effect on 1 September 2025. Bill 9 received royal assent on 11 December 2025. The order for 16- and 17-year-olds took effect on 7 January 2026.
