NHS asks clinicians how to help adults who want to detransition

On 30 October 2025 NHS England asked healthcare professionals for evidence to help design a clinical pathway for adults who have already medically transitioned and now want to detransition. This is a call for evidence.

On 30 October 2025 NHS England published a call for evidence. It is trying to design a clinical pathway for adults who have already had a gender transition — hormones or surgery — and who now want to detransition.

Plain English: some people stop or reverse that medical path. The NHS says many of them still have physical and mental health needs and struggle to get the right help. In April 2025 it heard from people with that lived experience. This later call asked doctors and other clinicians what they see in practice. It ran eight weeks, from 30 October to 28 December 2025.

NHS England uses the Cass Review’s definition: detransition is discontinuing or reversing a gender transition, often after a change in how the person understands their sex or gender. Regret is not required. Someone can detransition with or without regret.

Cass recommended that the NHS make provision for people who choose to detransition — including those who do not want to go back to the same gender clinic. NHS England is developing that pathway. A call for evidence is the first step toward that service.
