The Department of Health and Wellness Gender Affirming Care Policy (version 2, effective 15 April 2026) is the provincial rule. Appendix A lists criteria for puberty-blocking agents, including that the adolescent has reached Tanner stage 2. Appendix D says adolescents who have reached Tanner stage 2 need a biopsychosocial assessment before opposite-sex hormones. Publicly funded surgery is generally from 18, with an exemption process from 16. The policy names prideHealth as a partnership of the department, Nova Scotia Health, and IWK.

IWK Health is the public pediatric pathway. Its page says a transition may include puberty blockers, opposite-sex hormones, and surgeries, and that youth can self-refer through the provincial mental-health intake line or be referred by a doctor, nurse practitioner, pediatrician, or parent.
